ISHN Membership Application/Registration
Please complete the form below and press submit. You will be sent a PayPal invoice which you can pay by credit card or with your own PayPal account. That invoice will include any applicable taxes and a PayPal processing fee, which we use because of it's security in processing financial transactions. Please refer to this description of types and benefits of ISHN membership (individual @$60.00), organizational (up to 15 people@$250) and country/state (varying in size and ability to pay from $2,500 to $25,000) per annum. By completing this application for membership, the applicants hereby agree to the Terms and Conditions of such membership as described in the page Types and Benefits of ISHN membership. 
Notes:
1. ISHN will contact applicants for country or state memberships directly in regards to payment of the applicable fees. The size of the fee to include all designated people or organizations from these countries, states or provinces will vary according to their size and capacity to pay. A specific procedure for ISHN members to access ISHN services from these jurisdictions may also be established by ISHN and that jurisdiction. (This web site, like most others, can identify the location of anyone completing this form, so we will be able to process individual applications from jurisdictions who have purchased a country or state/provincial memberships without additional administrative steps being necessary from the member jurisdiction) 
2. ISHN is establishing service delivery partnerships with this list of international, country or state/provincial NGO's whereby they will receive a part of the ISHN membership fee for referring potential new members and for providing additional or customized services for individuals or organizations in their respective jurisdictions. (For example, the Canadian Association for School Health will receive 50% of the ISHN member fees in return for providing blog-based analysis of ISHN content and a Canada-specific Twitter feed.)
